ADOPTION FORM
Adoption Guide - follow these steps!
1) If you plan to adopt, you'll be asked to complete an application.
2) Bring a photo ID.
3) Applicants must be 18 years of age and older.
4) All applicants will be interviewed by an adoption counselor.
5) All applicants must agree that the animal they wish to adopt will not be housed outdoors.
6) Renters must bring a copy of their lease.
7) If living rent-free with a relative, said relative must give permission for the adoption.
8) Adoptions, specifically dog adoptions, will require a property check and/or a home visit.
9) A vet check on the individual's current pet(s) that show they are altered and up-to-date with immunizations.
10) On-site meet n’ greet with the prospective adopter's current dog if adopting a shelter dog.
11) Ability to provide a stable environment; proper veterinary care and meet the basic needs including food, exercise and companionship.
12) Payment in full at the time of adoption by cash, debit card, credit card or personal check.
13) Agreement from the adopter to contact Animal Protectors to make arrangements for the return of the animal if the adoption is not successful.
